Fungal Infection Treatment

In the last decade, recurrent and treatment-resistant fungal infections have become an epidemic across India — and Tirunelveli's hot, humid climate makes the problem particularly severe. Patients arrive at Orange Skin Clinic having tried multiple over-the-counter creams and combination steroid-antifungal "magic" creams that initially clear the rash but eventually make it dramatically worse and harder to treat.

The core problem is two-fold. First, steroid-containing combination creams (sold freely without prescription) suppress the immune response, allowing the fungus to grow under the surface and spread. Second, the dominant fungus in India — Trichophyton indotineae — has developed widespread resistance to standard terbinafine, requiring different drugs and longer courses.

We begin with KOH microscopy and fungal culture of skin scrapings to confirm the diagnosis and identify resistance patterns when needed. We then prescribe the right antifungal at the right dose for the right duration — often itraconazole at higher doses for 6–12 weeks, sometimes combined with topical agents and oral griseofulvin in selected paediatric cases.

Equally important is household decontamination — fungal spores survive on towels, undergarments, bed linen, footwear and combs, causing reinfection. We provide a written plan covering hot-wash temperatures, antifungal powders for shoes, and family screening (asymptomatic carriers in the family commonly cause recurrence).

We also stop all combination steroid-antifungal creams immediately — these are a leading cause of chronic resistance.
