What is Pediatric Scabies Treatment?

Scabies in children is caused by the Sarcoptes scabiei mite burrowing into the skin. It is intensely itchy — especially at night — and unlike in adults, children often have involvement of the palms, soles, scalp and face in addition to the typical sites (between fingers, wrists, armpits, waistline). Multiple itchy family members is the classic clue.

Otherwise reinfection is almost guaranteed.

For children we use 5% permethrin cream as the standard regimen — applied from neck to toes (and including the head and scalp for infants and young children, which is often missed in incomplete treatments), left on for 8–14 hours, washed off, and repeated after 7 days to kill newly hatched mites. Oral ivermectin is used selectively in older children with extensive or crusted disease.

Equally important is decontamination: all clothing, bed linen, towels and undergarments worn in the past 3 days must be hot-washed and ironed, or sealed in a plastic bag for 7 days. The mite cannot survive away from the body that long.

We also explain to parents that the itch can persist for 2–4 weeks even after successful treatment — this is an immune reaction to dead mite particles, not treatment failure. We provide antihistamines and a short topical steroid course for symptomatic relief, plus emollients to repair scratched skin.
